Doing this with a Mailchimp signup form.

I've looked around StackOverflow, but most questions relate to just checking if a certain checkbox is selected. In my case, I'd like to check if a certain checkbox is selected, the user has to fill out a certain part of the form (mailing address) which is multiple fields.

I've created a JSFiddle to hopefully explain it better.

So if they tick the 'Print edition' you want to show the mailing address section? otherwise it remains hidden? –  Mark Walters Apr 20 '12 at 15:06
    
Mailing address should always be shown (I will put a note that it's only required for print copies). If they select the Print Magazine checkbox, they are required to have all the fields related to receiving something by mail filled out or the form will not submit. –  Mike Last Apr 20 '12 at 15:47
