You should probably check that $email_address is a valid address, otherwise you will be vulnerable to email header injection –  Tom Haigh Jun 23 '09 at 12:27

mail()

http://uk3.php.net/manual/en/function.mail.php

The return value of this function is a bool. You should use that to determine if the mail was sent successfully.

You may want to check out the double quoted strings and heredoc string:

$name = 'Benedict';

$greeting = 'Hello ' . $name . '\n Would you like a cup of tea?';
$greeting = "Hello $name \n Would you like a cup of tea?";
$greeting = <<<GREETING
Hello $name
Would you like a cup of tea?
GREETING;

All 3 $greetings are the same.
