i have a table with form elements which the user fills in[date, reason, notes]. what i need is if the date is selected then the reason or notes has to be there, if it is there then the same has to be entered in the DB, else it should not enter in the DB.

any help will be appreciated

You need this validation on server side or on client side (browser). Search google for PHP form validation or Javascript form validation, depending on the case. –  lorenzo-s Jun 5 '12 at 8:41
