I want to make a form in parent (main) window with some fields that will have a hyperlink beside it, and when this hyperlink is clicked, it'll open a "modelbox" popup with options to fill that field. So I'll click in some option in this modelbox popup and it value is filled to parent window respective field.

My question is how to do this?

Can anyone give me an example code to do it?
