I tried to insert data using the following code, but I really I don't know how can I get result and what is the problem in my code, I tried to build firstly my form then I write the insert query and send it to a new form , any HELP please : Also I Have now the following error:

Welcome to Stack Overflow. Please don't use the mysql_* functions, they are no longer maintained and community has begun the deprecation process . Instead you should learn about prepared statements and use either PDO or MySQLi. If you cannot decide, this article will help to choose. Unfortunately, the entire setup is a pretty large mess.

One large problem is this line:

$sql="INSERT INTO findings ... ";

You defined a string that would take all of the values from the form, but you don't do anything with it.

To run an SQL query, you use mysql_query().

mysql_query($sql);

However, I would recommend reading up on the basics of PHP/MySQL so you understand the code you're writing. It looks like you're sticking things together, hoping it works.

Please don't use the mysql_* functions, they are no longer maintained and community has begun the deprecation process . I think You used two forms and even you used one form in another form like:

    //Some code
<form id="form2">
    //More some
</form>

It is never possible to use nested forms.That's it.if you use two forms then better to avoid nested forms and also give individual triggers for both.
