This how I'm filling my form with data from my database. I'm lost when it comes to inserting this dynamically created form in my database. Can you please help me with that?

I'm working in Dreamweaver by the way. Thanks!

EDIT

I found this code on a forum. It was supposed to insert the "id", array of two answers and date_time. I included and somehow got the answer and it's "id" but I don't know how to include my other variables. Can someone PLEASE solve it with this or another way? I'm totally lost and don't even know how this worked..

Please DO NOT USE mysql_query in new applications. This interface is extremely dangerous if not used correctly, and is being phased out of PHP. Using mysqli or PDO will ensure you have proper SQL escaping and compatibility with future versions of PHP. –  tadman Aug 23 '12 at 18:01
    
Have you thought about using a PHP framework to help creating records? CakePHP and CodeIgnighter are two popular frameworks that make adding records much easier than doing it by hand. –  tadman Aug 23 '12 at 18:02
    
This is a school project and it won't see the Internet. And I'm still a year into PHP mostly with Dreamweaver.. I know about frameworks but I haven't started using one, and it won't help me now because this is almost finished project... But thanks for letting me know! –  Mite Stojanov Aug 23 '12 at 18:15
    
It's worth the time to learn the correct interfaces as using mysql_query is doing it 1990s style and leads to some very bad habits you will have to later un-learn. –  tadman Aug 23 '12 at 19:10
    
Well I'll get to learning when I finish this. But I cannot edit all my queries right now, and I know I won't need to. My teacher uses even simpler and unsafe way to deal with queries... –  Mite Stojanov Aug 23 '12 at 21:36
