I need to forward html form values to a different websites form, the user inputs their First, Last name, and Email then the on submit it should open a new tab and link them to a different websites form and transfer the info they had already submitted onto the form leaving them only a few more spaces to fill out.

This is what I have so far, if you can please help and explain what you did it would be greatly appreciated.
