I would like to have an OK/CANCEL confirm window popup on a custom toolbar in the Kendo UI grid for asp.net MVC3. Has anyone done this or know how to implement?

You should define you own toolbar action see here, bind a function to it and there generate that KendoUI confirmation window.

If you have a toolbar definition like this:

toolbar    : [
    {
        name      : "confirm",
        text      : "Confirm",
        className : "k-grid-confirm",
        imageClass: "k-icon ob-only-icon k-i-custom"
    }
],

then you can bind a function using:

$(".k-grid-confirm", "#grid").bind("click", function (ev) {
    // Confirmation code
});
