Does Rails only include the application layout when it cannot find a layout named after the current controller? I think I've seen some conflicting behavior with different Rails versions.

If Rails chooses 1 layout to include (either the layout named after the current controller or the application layout if no controller layout exists), what about the situation where I have a standard template that I want all pages to adhere to , but some customization per page.

Duplicate everything in a separate layout for each page?

Yes for the first question. See Finding Layouts section of the Ruby on Rails Guides page on Layouts and Rendering to understand how layouts are selected.

In the situation where you have a standard template to use for all pages, with some customization per page, you can use Nested Layouts to reduce duplication with the multiple layouts.
