I am inexperienced in IIS and web site setup and am trying to run multiple apps under single site (called survey). So the apps will be accessed by urls like survey/reports , survey/sproj etc

For this purpose I created a site under iis7/windows7 named survey and set its app pool to .net 4 framework and set the physical directory to c:\inetpub\wwwroot . Then I have a project and I publish it using FileSystem option to directory C:\inetpub\wwwroot\reports

Now I try to access the reports apps(forms auth) using url like survey/reports and I get the IIS password prompt(see below) .Now if I change the physical directory of the survey site to c:\inetpub\wwwroot\reports , then I can access the reports app using url like /survey and it does the correct thing and takes me to the forms login page

Any help would be appreciated. For app pool I am using custom user and credentials instead of IUSR

what is the deploy mode of your Web.config file? if you right click on the file in solution explorer and go to properties it gives you a copy to directory setting. It's default is DO NOT COPY to prevent you from accidentally changing it. –  Dave Alperovich Jan 26 '13 at 22:50
