I have auth.php which has a link for me to authorise access to Google Analytics and get a refresh token, store it to a text file token.txt. I got the refresh token alright and saved it to token.txt alright.
Then I have test.php which picks up the refresh token from token.txt and attempt to access Google Analytics offline (so that I can show pageviews to visitors of my website). After $client->refreshToken($refreshToken), $client->getAccessToken() seemed to be successful as I could print out the access token e.g. {"access_token":"ya29.AHES6ZRZKhDxMMpTrzif-xXaTQMhYNTlFLu1hQgdb45b0sZ8w8eSJQ","expires_in":3600,"created":1361934280}

However, when I tried to send a get request, I got

Error calling GET https://www.googleapis.com/analytics/v3/data/ga?ids=ga%xxx&start-date=2012-01-06&end-date=2013-02-27&metrics=ga%3Apageviews&filters=ga%3ApagePath%3D%3D%2Fsome-article%2F&max-results=1&key=AIzaSyC6G0bDA-zsBGKRAHEj4JnSgF95dt_nE6g: (403) User does not have sufficient permissions for this profile.

Copied and pasted the GET url onto my browser, I got

I tried using Google OAuth 2.0 playground too, at Step 2, I enter my refresh token and click "Refresh access token" and I got unauthorized_client error.

What am I missing here?

It seems you have not token assess or after getting token access user has barred you from getting data. Kindly go to google.com/accounts/b/0/IssuedAuthSubTokens?hl=en_GB and check the access is granted or not –  Vineet1982 Feb 27 '13 at 5:35
    
Thanks @Vineet1982, it seems to me that you're referring to either AuthSub or ClientLogin protocol for authentication. I'm using OAuth 2.0 however –  fuzzybee Feb 27 '13 at 7:24
    
Could you check that the $client->getAccessToken() changes after you do the refresh call? It's surprising if the refresh would work in your code but not the playground. Also, what's the original scope you asked for? –  Tim Bray Mar 8 '13 at 19:54
