I have this in my .htaccess file

## REWRITE DEFAULTS ###
<IfModule mod_rewrite.c>
  Options +FollowSymLinks
  RewriteEngine on
  RewriteBase /
  RewriteRule lenna/(.*)$ /lenna/output.php?$1= [NC,L]
</IfModule>

I am trying to make:

http://nickkoranda.com/lenna/311x123

translate into

http://nickkoranda.com/lenna/output.php?311x123

but it is not working. I get a 404 not found error. I have tried this (http://htaccess.madewithlove.be/) online .htaccess tester and it passes the test there and shows the rewrite working correctly.

What am I missing here?

Make sure .htaccess and mod_rewrite is enabled on your server. –  anubhava Mar 1 '13 at 3:41
    
I have LoadModule rewrite_module modules/mod_rewrite.so uncommented and the RewriteEngine On as shown above. I believe it is enabled. –  SpecialK Mar 1 '13 at 3:53
