For a managed_file '#upload_location' - what should this be?
No matter what I enter here, I get 'destination is invalid' error.
I've tried an absolute location on the server: '/home/.../mysite/public_html/sites/default/files' as well as values similar to here: Drupal managed_file error on submit
from the drupal api documentation:
"$destination: A string containing the URIa $source should be copied to. This must be a stream wrapper URI. If this value is omitted, Drupal's temporary files scheme will be used ("temporary://")."
Is there more info about this type of destination format somewhere?
here's the code I'm using:

The error I keep getting:
The file could not be uploaded, because the destination /home/phu5er/public_html/dev72/sites/default/files/claims is invalid. The file in the Evidence of ownership (e.g. scanned electricty bill) field was unable to be uploaded.

it seems that now the file is indeed uploading (I've checked it on the server and its there) but I am getting the following ajax error:
An AJAX HTTP request terminated abnormally. Debugging information follows. Path: /file/ajax/file1/form-H5EgFgklhg6otOSQY7F9dq39bVVeYHee98X4lGJebRs StatusText: n/a ResponseText: PetHostr Error The website encountered an unexpected error. Please try again later.
