What could produce such behaviour? On most pages, the moderator view shows up.

But there are some where it doesn't. I can still comment, though, as a regular user. moderator view is NOT available

All the pages are dynamically generated by the same code. The meta information, the fb-root div, the fb-comments div and the sdk load are always present.

I do have the fb:app_id set, I don't have an admin, just like they recommend in the comments documentation.

Also there is a

The SDK is loaded like the same documentation suggests:
