I have created an HTML form that submits using a php mail script. The problem i am running into is that the form submits completely blank once each day, at exactly one hour later each day. I am completely stuck and I am hoping that someone can help spot my issue.

I am pasting the code below, please note: the end location for the html form is within a page of a site using Adobe CQ5, the php script is out on a separate server and i have removed all validation to to check if that was the issue (it's not - it still submits blank with or without my validation code).

It's probably a bad bot. You should capture info about who/what submitted the form and look for a pattern. – John Conde May 7 '13 at 13:02

You have marked a few fields as required, but are not checking that they are actually filled. Take submit_name for example, you have it marked as required (*Name) but when the form is posted you do the following:

if(!isset($_POST['submit_name'])){$submit_name = " ";}
else{$submit_name = $_POST['submit_name'];}

This code actually gives it a value " " even when its not provided. I guess what you want to do is to prevent submit when its not provided.

if (empty($_POST['submit_name']) or empty($_POST['companyName']) or empty($_POST['companyLocation']) or empty($_POST['companyCity'])) {
  echo 'Not all required fields are set';

  //now there are several options. You can just end the page using exit and then the user needs to use his backbutton to get back to the form

Side note:

You are using isset which will return true even if the variable is posted, but with no value. Perhaps you want to check strings with empty() to make sure they also contain something. But dont use empty() when you expect a number that can also be 0.
