Does anyone know how the various screen readers interact with a modal window, ie: Thickbox? Do the contents of the modal gain the reader's focus after they click on it?

It is functional with JS turned off, the modal links simply open in a new browser window in that case. But I'm not sure I can make the assumption that all users with screen readers will have Javascript disabled? – smurfarita Oct 30 at 18:30

Maybe. Javascript limits what screen readers can do. Some screen readers may watch for a css change in an item for when it becomes visible then reads it. The majority reads what is visible on page load.
