@Dagon really? there's nothing wrong with using UTF-8 as charset. – Raptor May 23 '13 at 3:09

Edit the target location of move_uploaded_file():

move_uploaded_file($_FILES["file"]["tmp_name"], '/var/www/images/' . $_FILES["file"]["name"]);

but is the file successfully uploaded to that folder? if not, is the file size exceeded max_upload_size of PHP or max_post_size ? – Raptor May 23 '13 at 3:08
