I am digging deep to achieve this. I have some experience in making custom Facebook iframe apps (but 'm a beginner). But they are basic html pages, showing some static html content, showing some information, like images, embedded content etc.

But now, I have an HTML or PHP form. I want to display this form in my page, in a tab. In that form, I have form fields like Facebook ID, name, email, phone, address etc. Is it possible to auto fill these fields using the user's Facebook info.

Of course it is possible (weel, for some of those fields - you won't get a phone number or an address, FB does not give this info via API). You have to have the user connect to your app and request the relevant permissions while doing so. If you have no idea where to start, go to FB docs. – CBroe Jun 28 '13 at 8:39
