SalaryCalculatorModel.php code above generates the and fills the columns on SalaryCalculator.php.

I want to save all the information on the rendered page (after I enter / calculate) values using SalarySaver.php

The question I have is I have form fields on Salary Calculator.php as well as columns filled out from the SalaryCalculatorModel.php. How can I save values on the dynamic html table into a mysql database table?

I don't know if i understand the question. You want to insert data from this table into a mysql db table? If so you should have

<form method="POST" action="page_where_inserting_in_mysql">
    ...
</form>

And into the new page you should have

mysql_connect("localhost", "username", "password") or die(mysql_error());
mysql_select_db("db_name") or die(mysql_error());
mysql_query("INSERT INTO table_name (field1, field2, ...) VALUES('".$_POST['value1']."', '".$_POST['value2']."',... ) ") or die(mysql_error());
