Honestly, don't use a paypal integrated button. Keep a session file and each time the page loads, it appends to the session. So in your php say roughly this:
and then each time a user adds a new item to the cart, complete the ajax request to that php file.
It should be fairly understandable how the ajax request should be formatted.
This takes a little more work than using a paypal cart, but you also learn more in my opinion and you can do some unique stuff that you wouldn't be able to do with just a paypal cart.
