What if a coder working for an open source project or inside a company wanted to slip some malware code into a release? Has this ever happened?

Yes, but the NSA's not saying! For agencies or organisations with deep pockets, it is indeed sadly likely that some obscure bits of little-read code in large projects have been placed there for the "benefit" of particular people. –  Nicholas Wilson Sep 16 '13 at 22:16
