I have a client who is asking me to develop a web app that consists of a series of web forms that will be filled in by customers and/or service technicians, which will be stored in a MySQL database. Then he would like the database to be synced with a desktop application in order to be searched and viewed, and perhaps even added to. He would like to stick with open source, but not necessarily free.

My first thought was to use Form Tools (formtool.org) to collect the data, then perhaps use a desktop MySQL viewer/editor for the front end. The main requirement here would be that the desktop app would be fairly easy to use for the client.

I appreciate any ideas or experience you might have in this.
