I have an audio stream that will play, then KVO observes playbackLikelyToKeepUp twice, then stops playing instantly observing playbackBufferEmpty twice. Firstly I don't see why observeValueForKeyPath should be called twice for each message. And secondly, the presence of playbackLikelyToKeepUp should indicate playbackBufferEmpty shouldn't happen.

I should note this is happening in the simulator and the device.

Are you actually checking the result? The playbackLikelyToKeepUp is a BOOL so it could very well be NO.
