My website redirects from http://example.com/ to http://example.com/#/

I entered http://example.com/#/ into "Website"/"Site URL" on the developers.facebook.com/apps "Settings" page. When I go to the "App Details" page, click on "Web Preview" and click on "Visit Website", it redirects me to:

http://example.com/?fb_source=appcenter&fb_appcenter=1&code=AQDPu8…long…cryptic…code…fKhl7N8I#/&

My #/ is moved to the back of the query string, which is not what I want. I want:

http://example.com/#/?fb_source=appcenter&fb_appcenter=1&code=AQDPu8…long…cryptic…code…fKhl7N8I&

Can anyone tell me what needs to be done to get this?

I want http://example.com/#/?ref=app_directory&code=AQBwnGy3...long...cryptic...code...1XC8VMghgw&fb_source=appcenter&fb_appcenter=1& though I'm not sure what the purpose of the long cryptic code is.

For my test Facebook app, I entered http://localhost:8888/#/ into "Website"/"Site URL" on the developers.facebook.com/apps "Settings" page. However, when I go to the "App Details" page, click on "Web Preview" and click on "Visit Website", nothing happens. The button is dead.
