The accepted answer for this question is for a previous version of TweetSharp, and the other suggestion there shown below, appears to return '20' for any account.

IList<TwitterUser> twitterFollowers = null;
ListFollowersOptions options = new ListFollowersOptions();

options.ScreenName = screenName;
twitterFollowers = service.ListFollowers(options);

int followersCount = twitterFollowers.Count
