I have "installed" URLRewriter.net to my website, and it works just fine when I run it from Visual Studio, but as soon as I upload it, I get a 404.

Example: http://dkbyg.strandweb.dk/FAQ

Application Pool has been changed to "integrated", because my host suggested that could be it. It didn't work though.

Any ideas?

I just had the same issue on my localhost install, but in my case switching the application pool to the integrated pipeline mode did resolve the issue. I suspect it is related to how the url rewriter plugs into the IIS pipline. –  Daniel Ballinger Apr 29 '10 at 2:05

I found the reason. I had to set the web.config as described in the section "Approach 3: Using an HttpModule to Perform Extension-Less URL Rewriting with IIS7" in this article: http://weblogs.asp.net/scottgu/archive/2007/02/26/tip-trick-url-rewriting-with-asp-net.aspx
