i want to do when user go to this page the javascript form will be auto-open,user don't need to click some button.

Just trigger a click event on one of the links. You won't have to write any new code, except for this one line:

    // Add this last

This code clicks the first a[name=modal] automatically, so you won't have to modify and create new code just for auto-loading this thing.

Try adding this code:

just take away the $('a[name=modal]').click(function(e) { ... }) wrapper - and you might as well take away e.preventDefault() too as there is no event to prevent.

right now, user need to click form button then the pop up will be appear, but i just want user go to this page and the pop up automatic open. –  Muhammad Rusly Mar 26 '11 at 1:47
I understand - just remove the code I specified, so the inner part is exposed. It will run automatically on loading of page. –  Billy Moon Mar 26 '11 at 2:15
