I just wanted to see how secure this form is, and if there are any potential problems. I tried to add mysqli_real_escape_string to the Prepared statement but it gave me an error.

Also if I enter a name with an apostrophe, like "Drew's Company" it puts it in the database as

Drew\'s Garage 

Is that how it should be?

Having a 16 char max for passwords is silly. If anything this makes your system less secure. –  Rook Aug 21 '11 at 17:43
    
Anyway other than my 16char complain this looks fine, but if you look at your response on SO, you have people just suggesting vulnerabilities. –  Rook Aug 21 '11 at 17:45

The problem might be caused by using magic_quotes_gpc, check your php.ini file and in case this flag is on - turn it off.

-1 magic_quotes_gpc is a horrilbe at stopping sqli, he is using paramterized queries which is the correct way also magic_quotes_gpc is being removed in php6, because its insecure. –  Rook Aug 21 '11 at 17:44
    
I didn't suggest using magic_quotes_gps, vice versa I am aware it is evil. I just thought that the problem might have been caused by <i>using</i> magic_quotes_gpc - and suggested Drew to check if this option was on (which could be the root of the problem) –  itsmeee Aug 22 '11 at 11:21
