Which runs on PHP, MySQL and does not require 3rd party software.

Define what you mean by 3rd party software. Depending on your definition then every bug tracking software is 3rd party, unless you've written it yourself. JIRA is good though if that passes your requirements. – Blair McMillan Sep 14 '11 at 8:09

@ilhan bugzilla doesn't need SVN, does it? It's an optional extra, isn't it? I daresay every good bug tracking software has SVN and other VCS connections but most don't require them – Pekka Sep 14 '11 at 8:15
