How to specify the number of citations of a source and the pages where are founded in bibliography of a Word 2007 document?

I use Harvard - Leeds style form http://bibword.codeplex.com/, which I have customized a little bit to show like that:

BERNERS-LEE Tim, HENDLER James, and LASSILA Ora. 2001. The Semantic Web. Scientific American. 285(5), pp.34-43.

But how can I modify this style to show me for each source something like that:

BERNERS-LEE Tim, HENDLER James, and LASSILA Ora. 2001. The Semantic Web. Scientific American. 285(5), pp.34-43. Cited 4 times in pages 48, 55, 189 and 211.
