When I press the user location button on iPhone Simulator (Xcode 4.2) it returns zero values for latitude and longitude. Then I turn it off and press button again. This second time requests the right values for custom location that I put in the Simulator -> Debug -> Location.

I just call

locationManager = [[CLLocationManager alloc] init];
locationManager.delegate = self;
locationManager.distanceFilter  = kCLDistanceFilterNone;
locationManager.desiredAccuracy = kCLLocationAccuracyHundredMeters;

[locationManager startUpdatingLocation];

annotation = [[[MapViewAnnotation alloc] initWithTitle:@"I'm here!" andSubtitle:@"Subtitle" andCoordinate:coord] autorelease];
NSLog(@"lat: %f, long: %f", coord.latitude, coord.longitude);

[self.mapView addAnnotation:annotation];

inside the turnOnOffUserLocation method which is called when I press the user location button.

I implemented the locationManager:didUpdateToLocation:fromLocation: like this

-(void) locationManager: (CLLocationManager *)manager didUpdateToLocation: (CLLocation *) newLocation fromLocation: (CLLocation *) oldLocation
{
    coord = newLocation.coordinate;
    [locationManager stopUpdatingLocation];
}

where coord is an instance variable. Is there a way to make the attribution to coord only happens when the right coordinates are ready?

Create and add the annotation in didUpdateToLocation. Location won't be available right after calling startUpdatingLocation. –  Anna Nov 5 '11 at 2:20
