i am trying to get multiple text boxes to appear based on the number of modules in a table so i can rate then and submit the ratings back to the database. I'm not sure how to implement this as. As the error i am getting is Parse error: syntax error, unexpected '<' in /Applications/XAMPP/xamppfiles/htdocs/cm3028/myFinal/choices.php on line 36

You must quote text (HTML) you're outputting. print '<form ...'; –  deceze Dec 3 '11 at 14:55

The error is in:

print <form action= "<?php echo $_SERVER['PHP_SELF']; ?>" method="post" >

You can't just mess HTML and PHP code together. You need to print the HTML like a string.

You can also write this code this way:

<?php
...
foreach ($results as $row ){
?>
<form action="<?php echo $_SERVER['PHP_SELF']; ?>" method="post" >
<label for="search">Rate 1:</label>
<input type="text" name="moduleID" id="addRate" value="" />
<input type="submit" name="submit" id="submit" />
</form>
<?php
}
...
?>
