Looks like a file permission issue. Try this:

# sudo chown -R couchdb:couchdb /opt/local/var/lib/couchdb/ /opt/local/var/log/couchdb/ /opt/local/etc/couchdb/

Try to run CouchDB as 'sudo'.

sudo couchdb

If chmod -R doesn't work, check the permission of parent directories.

My error was about /opt/local/var/lib/couchdb/_replicator.couch. /opt/local/var/lib/couchdb was fine (755), but /opt/local/var/lib/ was set to 750 somehow. sudo chmod 755 /opt/local/var/lib/ worked.

Had similar problems after building and installing from source on a Ubuntu 12.04 LTS. Starts up fine after reboot.
