Definitely a wiki. Although it could be some other system as long as it supports the following:

  • Simple and fast to make changes/update.
  • Ability for developers to record information without major context switch.
  • Powerful search.
  • Automatic history tracking.
  • Support for diagrams.
  • Accessible from offsite/home.
  • Reasonably rich formatting capabilities.

For smaller teams, organisations TiddlyWiki might be one of the simpler and easier options. It works best if you have separate files per project, but combined with Firefox/Canvas it meets most of the points mentioned above.
