I write it in paper and then use a software like Visio.
Firstly work only the use cases direcly related to the users. Use them to start designig some interfaces. This interfaces are your main tools for finding new requirements and showing progress.
Most of the actual content of the diagram (like extents extends and includes) should be written a little further in the proyect, while making some technical and architectural choices. In that way the document can "guide" you, and doesn't "force" you that much.
You also must avoid falling into a kind-of-Waterfall development, a common error when using formal aproaches (the latter guideline can help you with that).
