You don't need to worry about the numbers of columns, you just provide the number of rows i.e. n=4 for this example. At the end of every 4th line a new column is created (12/4=3).

Good tip @glennjackman my underlying assumption is the file is sorted lexicographically already so you could just throw it through sort after the awk to get the same ordering. Another benefit of this approach is the file doesn't need to be sorted for it work.
