@Casper Damn - that was the other option. :-) Incidentally, generally accepting the "first to answer" isn't the best approach - the ideal for StackOverflow is that more detailed/fleshed out answers should rise to the top. That said, in this instance there's not really much between them.

@AustinBunker To use the include statement, the page must be parsed as PHP. (Most likely this will mean it has a .php extension on your system.)

@AustinBunker In the header.php file, there's no need to have opening and closing tags (i.e.: <?php and ?>) - these are only necessary if you want to use execute PHP. Incidentally, I'd recommend that you read the PHP tutorial as it should clear some quite a bit of confusion for you.

@AustinBunker In the header.php file, you can use both HTML and PHP. (Just because the file extension is .php doesn't mean it has to contain PHP - it just means that the web server will look for PHP code inside it.)

@Jay Change the REPLACE(TRIM(city_name), " ", "-" to REPLACE(TRIM(city_name), ",", "-". Incidentally, if you don't start reading the manual, you'll never learn anything.

@Jay Er... what? You can use the logic above to replace spaces with dashes in any column. (You can also update a column with a version of that column with no spaces, if that makes sense based on your data.) i.e.: UPDATE city SET column_a=LOWER(REPLACE(TRIM(column_a), " ", "-")).
