Can you identify one of the records that the workflow hasn't fired on. Open that record. Check the running workflows on the left nav of the open record. Is this workflow present (and Waiting?) or is it not present at all? If its waiting, open it and see why it stalled. Is the country a required field?

It does appear that on the User record that you have the option to set that users' default queue. Presumably this could be an existing queue or one that was recently created. This, to me, answers my original question. Can I delete (default) Queues for users that are disabled. Then later if the user is reactivated a new queue be created and assigned to be their default queue.

your most recent comment is consistent with my experience and testing. And my interest is in the sizable number of "dead queues" that stack up. My preference would be to deactivate. My experience is that deactivating a queue does not seem to remove it from the drop down.

Also, you say that it must all be client side. Would you consider going through an intermediary service? Amazon SQS or SNS as a message bus? Pull messages on the CRM side? Or you could ask SNS to POST to a custom webservice on the CRM box.

You may want to consider this link. There's also this [link] (blogs.msdn.com/b/girishr/archive/2011/02/04/…) that talks about how to access the Online webservices via SOAP. Maybe some of that can transfer to what you are doing?

... and I'm not the best source for notes on Internet Facing deployments. Forms Auth is probably in play. I think you will find that unless the submitter is preauthenticated to CRM, the requests will fail. I've not found a way to provide authentication (other than Windows Live accounts to CRM Online) to external uses of the REST service.
