Exactly. Since the files have a large variance in size (200mb to 1gb, etc..) it is best to employ a method that works regardless of the size of file.

@BenD Generally speaking, I always implement those techniques when dealing with big data. This particular script is a one-off for one specific task that I will only perform once or twice. So I decided to take some shortcuts. It seems that bit me in the behind.

@BenD After struggling with this long enough, I am coming to that conclusion. I am running this on an Amazon cloud server, and perhaps it is behaving oddly.

@datasage I would have assumed memory if it peaked somewhere in the middle of the loop, but the memory peak happens way before the while loop executes. No new memory (non-trivial) is allocated during the loop.

Doing a quick test, it seems it is not any of the css classes or ids. I would first remove this script: <script type='text/javascript' src='http://www.endingpovertytogether.org/wp-content/plugins/mailchimp/js/mailch‌​imp.js?ver=1.2.10'></script>, and see if it fixes the no submit problem.
