You may want to take a look at IX509CertificateRequestPkcs10, specifically InitializeFromPrivateKey and IX509Enrollment. I know I have seen other examples on SO, though.

From the API docs: "Message.Raw: The entire email message in an RFC 2822 formatted and base64url encoded string. Returned in messages.get and drafts.get responses when the format=RAW parameter is supplied.". Looks like you need to request it in Raw format to get the full message.
