Okay - you updated it to injury_details_under but are trying to grab it using injury_nature_under - if it's not another copy error, that could be the issue.

you need to give your <select> a name value on the ajax_details.php page or it won't be submitted. Change it to <select name="injury_nature_under">

You could be blocked by spam filters - try adding "-f your-email@example.com" after the $headers - so ... $headers, "-f your-email@example.com"); This will make sure that the email is sent from you and not the apache server (or at least mask it that way). Ultimately, I would suggest using something like PHPMailer instead of the basic mail() function

I don't know if this will help, but I had the same issue when trying to create temp. files on the server - it turned out that the host had turned on FastCGI on the server - it doesn't handle memory the same as regular Apache. I would look into that and make sure that's not the case with you as well - turning off FastCGI fixed all my memory issues.

That's incorrect. if you add [] to the end of the radio name, it will take the as long as there is a number for each radio button. So, if the first radio name is answer_grp1[0] and the second is answer_grp1[1] - then it will be all be contained in the same array at different indexes.
