Your question is not really about PHP. It's about HTML. PHP just happens to be the code you are using to generate the HTML, but you could just as easily be using Ruby or Perl or just writing static HTML directly and the question would be just as applicable.

You need to add more detail. Do all buttons need to link to the same URL? Do they all have the same values? Do they all need to submit the same data fields? Do you have the option of using Javascript?

I see the same intermittent issues. Sometimes after days of successful 200's, I'll start getting 403's from one device while continuing to get 200's from another. The object wasn't recently updated. The device clock time is correct. Any ideas?

In version 4.5.0, I managed to do it using FactoryGirl.configuration.sequences[:iterator].instance_variable_get("@value").‌​instance_variable_set("@value", 1) (where ':iterator' is the name of the sequence). Note that this only works on global sequences.

#to_file was removed in paperclip 3.0.1. In that and later versions, use Paperclip::Geometry.from_file(Paperclip.io_adapters.for(photo.styles[style]))

2) Have Rails log the full environment on each request. You'll need to create a simple piece of middleware that just logs env.inspect before passing env to the next layer of middleware. See gist.github.com/betesh/275c55abe7436417177b

1) You can look in your server logs. For instance, if you're running Apache on Linux, look in /var/log/httpd/. You can edit your Apache configuration to log more verbosely until you've solved the issue. That way you'll be able to see the full request.

Which routes and which formats is this occurring with? If it's only happening on AJAX requests, that might explain the connection. If so, it could be that jquery is not formatting the data properly on those AJAX requests.
