Try build 793. It's possible that the 404 is actually a missing dependency. Newer builds unmask the underlying problem (in theory).

You should probably use IIS configuration rules or roll your own plugin for setting headers - ClientCache itself is only 8 lines (excluding boilerplate)

Anytime you change the value for 'subfolders', you will need to manually delete them. If it hasn't changed, then autoclean could do it for you. But renaming the folder, then deleting it after an app pool recycle is certainly the fastest way.

Yes, the &quality=80 command will let you adjust the compression. Keep in mind that each time you re-compress a jpeg, it gets larger - jpeg artifacts are hard to compress. The higher-quality the original, the smaller the result can be.
