The DOT.NET wrapper for this initializes the output to "" instead of null. If Okay is pressed the contents of the TextBox is assigned to this.Output (which may be an empty string ""), if Cancel nothing is assigned. Therefore you cannot distinguish between the two outcomes. Some people initialize the default value as " " as a work around.

At the server level (or server instance if you have multiple instances on a machine) the users must have been added (or added a login for the group or domain they are part of depending on how you want to manage this) with status of Connection Grant and Login enabled.. and then at the database level (which hold the FILETABLE) have those permissions to access the folder by the entire path. To be able to browse at \server\instance\filestream directory name\ they need addition permission of ALTER for some reason.

You must also check that your SQLSERVER instances actually is listening to the localhostIP (127.0.0.1) or all Adapter IPs rather than just listening on the external IP interface (ie your network adapter)

I'm using your code but having problems returns part of a JSON string where there is no subkeys. I am getting an unescaped command deliminated value for the entire "table". Any ideas?
