I did have the words ordered in my example, but that was just an old habit to illustrate the problem. Would there be a succinct way to preserve order if that was a requirement?

Edited answer... For sqlite, I don't believe the '0' is needed although it is generated by default. Because the soname is actually sqlite3 indicating the major version '3'. I haven't studied sqlite specifically, howoever I have seen this behavior form other libraries.

Are you able to enumerate types from other Get*Assembly calls? i.e. GetExecutingAssembly() or the like. Just trying to narrow down what the problem might be.

mod_rewrite routes the incoming request to your actual file location. It can be used to map full filesystem paths or for doing advanced URL rewriting. For your example, all requests to /contact would be treated on the server as if they were called by /index/contact. I believe this is what you want.
