I think both answers are right, they tell two sides of the same story. The message is shown when a request is blocked or the resources are loaded from the cache, but also after every request is initiated and while the browser is waiting for a response from the server. As soon as the response arrives, the message goes away and the real headers are shown.

I'm out of ideas here, so I'm just gonna ask simple questions hoping to hit something :). You are using a relative URL as the parameter to your Ajax call. That URL could be interpreted differently in production and development, depending on your configuration. Are you sure the request is being made to the right location?

The net::ERR_EMPTY_RESPONSE and Provisional Headers Sent - No actual request sent errors you have mentioned should be more related to your problem. Is there a possibility we can see your system running? It seems the problem is not in the code, but in the client-server connection.

I'm answering a comment you left here. I don't think your problem is the "Provisional Headers Are Shown" message. As far as I can tell, that message does not represents an error, just shows that Chrome hasn't received real headers for that request.

Sorry for the late answer. The code above is part of a WordPress plugin. I was seeing different results with the same source code, but different machines. Some users reported problems when the custom certificate bundle was used, and others reported problems when the default bundle was used. My solution was to support both scenarios: try with the custom bundle and repeat the request without it, in case of error.

You're right @AndrewSchools. Thank you. I thought about it after you made the question. So far it seems like switching to PHP 5.3.x removed the problem, but I'll keep an eye on server load if things go wrong again.

@Ohgodwhy I was able to switch to PHP 5.3.27 from the cPanel. I think you were right because I haven't been able to reproduce the error again since I changed the PHP version. I tested both with APC enabled and disabled. Thank you a lot. I think that's going to be the solution for us. If you post an answer I will accept it. I still would like to make it work in PHP 5.4 in the future. Do you have additional information about those bad modifiers used in preg_replace?
