The question is really how the dump was generated. My guess is that it's generated by WER (windows error reporting) which kicks in if you're trying to send an UI event to an responsive thread, in which case you can choose terminating the unresponsive app and a crash dump that results will have this as an exception recorded.

I believe it is correct - attributes are expected to be fields, if you stash them into a substruct, there're no longer on the same level hence the result. The question is why would you not use embedding as you had?

This is indeed strange. Can you verify the debugger process has loaded a copy of dbghelp.dll (in process explorer, for instance) or that dbghelp.dll from the debugger's folder is indeed a valid PE file (dumpbin or depends to the rescue)?

1003f is a value of CONTEXT's ContextFlags (take a look inside winnt.h), if you're lucky you can dig out and use to switch thread context to the context of the exception. It is stored by the system when it's running exception handling code. So if you run the search, you might get lucky and find such exception context, switch to it (.cxr <address>) and see what exception is being thrown from destructor of which object.
