@Mark, It doesn't say BEFORE the tracking snippet, it says before it is loaded which is entirely different. If you place the code above after the tracking snippet and it runs before the analytics.js file is loaded is works.

If Analytics is not tracing you are not including the plugin correctly. When you run ga('require', 'campaignLoader') analytics will stop processing further commands until the plugin is loaded and the ga('provide', 'campaignLoader', ...) is run. So probably that line is not running. Make sure your plugin is included. If the file is correctly included you should see the plugin inside the object window.gaplugins

You should update it to use the new version that is documented here: developers.google.com/analytics/devguides/collection/…

The php file seems to generate image content. You can't add Javascript to it. It's either a page with javascript or an image. You can't have both.
