You can't set them because the headers are sent as soon as ANY output is produced. Why? They are headers. They come at the head of the request/response! You can't change because it was already sent. How will you change a network package after it was sent but before it arrived? You can't!

I agree with most of what you said. But still, you really can't say "we can stop worrying about IE7". Sadly, there's still people stuck with Windows XP or even older (Windows NT). So, you can't really say it. For most cases, yes. For something specific, no. We don't know anything about the expected environment where the code will run since that information wasn't provided.

This saved me for a pretty weird Regular Expression for PHP and Javascript. In it, I had to match everything if it didn't had any whitespaces OR only match the first 1-3 non-whitespace characters. This is the result: /^(?:(\S{1,5})\S*|(.{1,3}).*)$/. It's ugly, I know. And it requires the replacement string '$1$2', but it works fine for the purpose!

I've picked this version and took it from here, to make a better version. Most of it is working fine. but I really had to do some heavy changes, including adding some styles. Also, this code blindly trusts whatever input is fed. It is so underperformant. You should save those tables[i].rows[j].cells[k] in a variable. Also, storings the length of the pieces in a variable will ease the processing time. I've, so far, made it run in less than 200ms for 2 tables, with over 700 cells.
