Correct me if I'm wrong, but it seems like, this method will say the form has been changed, even if someone makes a change and then "undoes" their change, manually reverting back to the original values in the form. In this case the form really wouldn't have changed with respect to the data, yet this method would say it had.

Your solution doesn't work if the captions are in text boxes. If you hit CTRL-A then F9, it won't update the field codes/numbers. It took me some time to figure this out. Putting them in text boxes is nice in a small document because you can group them and they'll float together, and I like top and bottom wrapping. But in a large document it's really not an option because of this issue, so you need to use paragraph settings to keep the captions with their content.

I'm leaning towards using stack suggested in another answer, because it's a base R function and as far as I can tell it does exactly the same thing in my case, without needing to include another package. It seems like melt is needed when you have more columns. Is there some reason to use melt over stack in this case?
